Sam Jackson
Doctor of Physical Therapy
Born and raised right here in Greenville, SC, Sam attended the University of South Carolina for his undergraduate degree in Exercise Science. He then headed down to Charleston to get his Doctorate in Physical Therapy from the Medical University of South Carolina. After working several years in Charleston’s largest physical therapy private practice, Sam set out on a mission to bring great physical therapy care to his hometown. Sam specializes in orthopedic and manual physical therapy.
He loves helping patients recover from back, shoulder, neck, and knee injuries. He believes in providing hands-on treatment and prioritizes clear communication with his patients. When he is not helping people overcome their pain and injuries, he enjoys spending time with his wife, son and extended family, traveling, gathering with the local church, hiking, and cheering on the Gamecocks and Las Vegas Raiders.
Paige Payne
Doctor of Physical Therapy
Paige spent her childhood and high school years in Greenville, SC. She then went on to the Lowcountry for her Exercise Science degree at the College of Charleston, where she stayed to earn her Doctorate in Physical Therapy at the Medical University of South Carolina. Upon graduation, Paige returned to Greenville and spent several years working in an outpatient orthopedic setting. A passion for sports physical therapy led Paige to complete an extensive Orthopedic Residency program to further sharpen her skills and better equip herself to bring the best care to her patients back here in her hometown. Paige is thrilled to be a part of the Foundation PT team, where she is able to put the patient first and provide quality, one-on-one therapy to those in need.
Though her name is Dr. Payne, she believes in providing quite the opposite experience for her patients. Paige enjoys utilizing an individualized, hands-on approach to empower her patients to enhance their quality of life through movement! She values compassion for and connection with her patients in order to help them reach their goals. Other than physical therapy, Paige enjoys spending time with family and friends, exercising, exploring, cooking, and learning new hobbies!
Olivia Jasinski
Doctor of Physical Therapy
Olivia was born and raised in Syracuse, NY. She received both her undergraduate degree in Clinical Health Science and Doctorate degree in Physical Therapy from Ithaca College in Ithaca, NY. After graduate school, Olivia moved to the southern coast of North Carolina where she spent the first 2.5 years of her career before moving to Greenville, SC in 2022.
Olivia believes her purpose is to be a light for those around her and enjoys making her patients feel cared for and heard as they go through their time in physical therapy. Olivia has worked with a variety of orthopedic diagnoses as well as neurological diagnoses such as balance related difficulties and Parkinson’s Disease. She has a passion for working with older patients and those that have difficulties with daily functioning and abilities. She is excited to be a part of the Foundation Physical Therapy team where she will be able to provide quality, individualized care for her patients helping them to succeed in their goals.
Outside of work, Olivia enjoys spending time with her family and friends, being a part of her local church and recreational sports teams, art, cooking, and finding new places to explore outdoors.
Forrest Rogers, DPT
Doctor of Physical Therapy
Forrest grew up in Greenville before attending Clemson University in 2013. After graduation, he relocated to Charleston to pursue his Doctorate in Physical Therapy from the Medical University of South Carolina. After receiving his DPT, he started working at a private practice in Charleston specializing in orthopedic & manual physical therapy. He & his wife moved back to Greenville in 2024 to work for Foundation, and we are happy to have him back in the upstate!
When it comes to orthopedics, Forrest is a jack of all trades. From the neck to the foot, he has extensive experience with both surgical & non-surgical cases. He has a passion for helping people understand their body’s dysfunction and placing them on a clear path to recovery. Forrest is also experienced in trigger point dry needling, which he has found to be transformative to rehabilitation.
While he is away from the clinic, he loves spending time outside or traveling with his wife, Rachael. He likes to keep himself busy with hobbies/activities which include coffee roasting, film photography, Crossfit, pickleball, working outside, and reading.
Hannah Martin, DPT, MS
Doctor of Physical Therapy
Hannah is a Greenville transplant, originally from Roanoke, Virginia, but fell in love with the Upstate when she moved to pursue her Doctorate of Physical Therapy at Anderson University and decided to stay after graduating in 2023. She attended Liberty University for her Bachelor’s degree before pursuing her Master’s Degree in Exercise Science: Performance and Wellness with a concentration in Nutrition. Hannah has a passion for integrating overall health and wellness to her practice, looking at systemic effects and addressing her patients in a holistic manner.
Outside of her career in Physical Therapy, Hannah loves spending time outside, whether rock climbing, hiking, playing pickleball, or going for a run on the Swamp Rabbit. She is an avid traveler and spent her summers as a wilderness guide on the West Coast during college. She is very involved in her local church and strives to be the hands and feet of Jesus in all she does.

Julie Bainbridge
Patient Care Coordinator
Julie was born and raised in Nevada and from there she lived in Colorado, near the amazing Rocky Mountains. She now calls Greenville home and absolutely loves living in the Upstate! She pursued a career in finance before becoming a mom, devoting her time to raising and educating her children.
Julie enjoys helping others and will be quick to greet you in the office and help you feel welcomed! She is committed to creating an excellent patient experience!
When she is not helping care for patients, Julie loves spending time with her husband and three children. She enjoys photography, pickleball, hiking, and ballroom dancing. She also loves serving her local community and church.
